Terms and Conditions

Please read our important terms and conditions:

• Prior to the beginning of any visit, a drivers license or state id will need to be provided.  In addition, our consent for services and clinic policy/procedures will need to be signed.

• When booking your appointment, you must pay the required deposit associated with the visit.  

• In order to provide all patients with the best experience, we ask that you provide a full 48 hour notice if you are needing to cancel your appointment.  If you are unable to do so, the deposit that was needed to book your appointment will be kept.

• Our provider values the time she is able to spend with each of her patients.  In order to make sure that she is able to provide exceptional service to each patient, she asks that you be on time for your appointment.  There is a 10 minute grace period but after the 10 minutes pass, your appointment will be canceled and the deposit that was needed prior to booking your appointment will be kept. 

• There may be times where our provider will need to discontinue services with a patient.  Bullying, hateful speech: such as foul, vulgar language, going against medical advice or constant non-compliance will not be tolerated.  In addition, if a patient chooses to do anything that would be deemed illegal: such as tampering with controlled substances, they will be reported to the local authorities.
